Systems thinking requires consideration for all parts of the whole, how the individual fits into
the larger community, how the various influences affect the individual, and how to best achieve the end
objectives. However, physicians may not consider the end objectives of long term good health, as they
work within confines of a system that does not seem to encourage optimal health as the end result. For
example, limitations on time spent with the patient, on what 3rd party payers will pay for a specific
procedure or visit, and the patient’s ability to pay all factor in to serve as limitations. After a period of
time, physicians tend to think more about the limitations rather than the possibilities. They give up
trying their best because the other parts of the system, insurance companies, government, and
healthcare organizations do not do their part to support the same objective (Story, 2013).
Systems thinking requires considering the broader picture. Patients live within societies,
communities and cultures that influence their lives. Patients face ongoing challenges in obesity related
diseases and others related to lifestyle, partly influenced by fast food availability and lack of physical
