Congestive Heart Failure (CHF) can be described as a chronic disease that has progressive debilitating symptoms often resulting in numerous inpatient admission and 30 day readmissions from poor self-management. Patient education is critical in providing patients with the tools to prevent complications and unnecessary hospitalization. Barriers to patient success in self-management include poor education of the diagnosis of CHF, symptom management and discharge instruction (Gosselin., et al 2014).Evidenced Based Education PlanThe education plan will improve patient’s ability to self-monitor CHF and promote follow upwith primary care providers post discharge. Evidence based CHF care includes left ventricular systolic heart function assessment, medication management with ACE Inhibitor, ARB or anticoagulants based on systolic dysfunction or atrial fibrillation, immunization with pneumococcal or influenza, smoking cessation counseling, and discharge instructions (Gosselin.,et al 2014). The education plan will be implemented at the time of admission to assure adequate time for nursing staff to work with patients in completing the curriculum. On admission, the EMR will flag patients with a diagnosis of CHF on admission to alert the care team to notify the heart failure care clinic team that a patient has been admitted to start implementation of the education plan (Gosselin., et al 2014). Once the heart failure care clinic team has been notified, they will evaluate the patient’s immunization status. Standing orders will be utilized to bring the patient up to date on influenza and pneumococcal if needed. If the patient is a smoker, the nurse will provide smoking cessation counseling and work with the provider to offer nicotine replacement therapy if the patient is interested and a referral to pu……………………….
